New Client Forms
We are delighted that you are considering Anoka Equine for your horse's veterinary service needs. Please call us at (763) 441-3797 with any questions you may have about our services and to schedule your farm or clinic appointment. Please complete the New Client Form prior to your appointment and return it to us via email or mail, as time allows.
We require payment at the time services are provided. We accept all major credit cards, cash and checks.
For those clients who are unable to be present when services are provided, we can keep a secured credit card on file. This can be a one time authorization OR if you prefer, we can keep it on file for future services. We will process your payment and then send you a receipt via email, fax, or mail.
